Cornerstone VNA Welcomes Katherine Berger As Transition Care Manager And Business Development Coordinator

KatherineCornerstone VNA is pleased to welcome Katherine Berger as Transition Care Manager and Business Development Coordinator for the Greater Seacoast region.  In her role, Ms. Berger will collaborate with case management, patients and their families to assure that patients understand their home and hospice care benefits and the scope of programs and services available at Cornerstone VNA. She will be part of a multidisciplinary team including physicians, registered nurses, physical, occupational and speech therapists to ensure the safe and appropriate care for all patients as they transition from acute care or rehabilitation to home care. As part of her effort to raise awareness of the extensive programs and services at Cornerstone VNA, Ms. Berger will provide community education and work to develop a strong network of relationships with health care partners, business leaders and members of the community.

Ms. Berger brings a wealth of experience to the position. Most recently she served as Director of Disability Services at University of New Hampshire in Durham, providing the overall leadership and management of a broad scope of services for diverse students with disabilities which including but are not limited to, physical, medical, chronic illnesses, learning and mental health disabilities. Working with her team, she served as a liaison advocating for individuals with disabilities within the campus community, as well as, through a variety outreach activities to the medical community including hospitals, orthopedic clinics and physician practices. Prior to that she held a position as the Disability Specialist and Testing Coordinator at Canisius College providing disability services to eligible students and a Counselor at Transitional Services, Inc., Amity House in Buffalo, NY providing recovery services and support in the community based residential setting. She is a member of the American Counseling Association and served as Vice President for Professional Services and Professional Development.

Ms. Berger received a Bachelor of Art degree in Psychology and a Master of Science degree in Counseling and Human Services from Canisius College in Buffalo, NY. She also earned a Certificate in Business Leadership Development at Canisius College and a Woman’s Leadership Development Certificate from HERS Wellesley Institute in Natick, MA.
