Telehealth is a technology program that Cornerstone VNA has used since 1998 to promote patient self-management and to reduce avoidable readmissions.  Cornerstone VNA invested in new telehealth units in 2016, in partnership with Health Recovery Solutions (HRS), in order to meet the demands and changes in the delivery of home health care services.  A powerful feature of telehealth is the ability for our Telehealth Coordinator, Anne Carle, RN, to video conference with patients at home.  Carle recently shared a great story about the benefits of Telehealth, which has been posted on the HRS Blog.  Click here to read Anne’s Story.

About the Telehealth Program at Cornerstone VNA:

Cornerstone VNA is proud to partner with Health Recovery Solutions (HRS) to provide Telehealth, which is a portable disease management system that is placed in a patient’s home to closely monitor chronic illnesses. The telehealth units are programmed and updated by a nurse to be specific according to the patient’s needs and doctor’s orders. Each patient receives training and support on using the system. Patient data is collected daily via the system, and other devices, and can include: blood pressure, pulse, pulse oximetry, blood sugars and weight. The unit may also include simple questions related to symptoms that the patient answers each day and clinicians can video call patients at home.

Cornerstone VNA Offers Free Hospice Volunteer Trainings

Cornerstone VNA is offering free trainings for individuals interested in becoming a hospice volunteer.  For as little as 2-4 hours per week, volunteers can offer companionship, respite care and their own unique skills to patients and family members in their community. This comprehensive 8-week Hospice Volunteer Training program focuses on how to provide comfort, support and a reassuring presence to Hospice patients and their families. No medical or volunteer experience is necessary to make a positive difference. Several training opportunities are being offered at various locations.

  • Wednesdays, January 10 – March 14 from 1–3pm at the Dover Public Library, 73 Locust Street, Dover, NH
  • Thursdays, January 11 – March 15 from 1–3pm at Cornerstone VNA, 178 Farmington Road, Rochester, NH
  • Tuesdays, March 20 – May 8 from 1–3pm at the Rollinsford Public Library, 3 Front Street, Rollinsford, NH
  • Wednesdays, March 21 – May 9 from 11am–1pm at the Strafford Public Library, 1151 Parker Mountain Road in Strafford, NH

Cornerstone VNA Hospice Team Receives Kind Gesture During National Home Care and Hospice Month

The Hospice Team at Cornerstone VNA was recently gifted with a kind gesture in recognition of National Home Care and Hospice Month. Lyzz and Kathy of Elements Massage in Portsmouth came to the Cornerstone VNA office to give the Hospice Team complimentary chair massages.

This special surprise was organized by Cornerstone VNA Social Worker, Hollie Crosbie, who said, “As Hospice workers we really need to keep that balance of giving of ourselves, but also taking care of ourselves.” Hospice and Palliative Care Director, Kelly Doherty RN-C, MSN, ARNP-BC, added, “Our Hospice team works tirelessly each and every day providing exceptional and compassionate end of life care for our patients and their families. We were delighted by this unexpected and relaxing start to their day by Elements Massage.”

Cornerstone VNA recently welcomed Melissa Sabina and Greg Hopkins to the Board of Directors.

Melissa Sabina of Bellamy Fields Assisted Living and Greg Hopkins of HBL Group, LLC

Ms. Sabina has been the Administrator of Bellamy Fields Assisted Living Facility in Dover, NH since 2012, which proudly serves residents with dementia, behavioral health concerns, and end of life care needs.  Prior to that, she worked as an Adult Protective Service Worker with NH Bureau of Elderly & Adult Services for 14 years, investigating reports of abuse and neglect perpetrated against disabled and elderly adults, and provided case management for individuals with ongoing needs for support in the community.  While based out of the Rochester Department of Health and Human Services office serving Strafford County from 2001-2012, she formed a close working relationship with staff at Cornerstone VNA as they collaborated on numerous issues regarding clients in common. Melissa completed her M.S. in Gerontology & Management of Aging Services with UMASS Boston in 2013, and is currently enrolled in a Leadership certificate program through Cornell University.

Cornerstone VNA Achieves Record Low Hospital Readmission Rate For Poly-Chronic Patients

Cornerstone VNA, a nonprofit home health and hospice organization, is offering free trainings for individuals interested in becoming a hospice volunteer. The training will begin on Tuesday, September 12th from 1:30 pm – 3:30 pm and will continue every Tuesday until October 31st. Join us for this program that focuses on how to provide comfort, support and a reassuring presence to Hospice patients and their families. No medical or volunteer experience is necessary to make a positive difference.

In addition to hospice volunteering, Cornerstone VNA is also pleased to offer a unique volunteer opportunity that involves caring for the pets of hospice patients. As a new partner with the national Pet Peace of Mind Program, Cornerstone VNA is recruiting volunteers with an interest in offering pet care, such as dog walking, clean-up and other helpful tasks. With support from Pet Peace of Mind volunteers, patients are able to complete their end of life journey without worrying about their pet’s current or future needs. To become a Pet Peace of Mind Volunteer, Cornerstone VNA is offering 3-week training programs in two different locations. There will be a training at the Newmarket Public Library on 1 Elm Street in Newmarket, NH, which will begin on Monday, September 11th from 2:30pm – 5:30pm and will continue every Monday until September 25th. Another training will be held at the South Berwick Library on 27 Young Street in South Berwick, ME, which will begin on Thursday, September 14th from 1:00pm – 4:00pm and will continue every Thursday until September 28th.
