The fair will take place at the Cornerstone VNA office located at 178 Farmington Road in Rochester, NH. Individuals interested in a rewarding career at Cornerstone VNA will have the opportunity to speak directly to staff about open positions within Home Care, Hospice Care and Life Care – Private Duty. Current job openings include Registered Nurse, Speech Therapist, Occupational Therapist, Licensed Nursing Assistant, Homemaker, Personal Care Service Provider, Hospice Registered Nurse, Hospice Certified Nursing Assistant, and Hospice Chaplain. Cornerstone VNA Achieves Record Low Hospital Readmission Rate For Poly-Chronic Patients

Cornerstone VNA of Rochester, New Hampshire, has again set the mark for excellent care with their highest-risk patient populations. For the six months ended June 30, 2017, Cornerstone VNA achieved a readmission rate of 3.4% over 146 of their high-risk, poly-chronic patients. This number far surpasses both the New Hampshire state readmission rate benchmark of 18.2% and the national benchmark of 16.7%.*

Cornerstone VNA Offers Free Hospice Volunteer Trainings

Cornerstone VNA, a nonprofit home health and hospice organization, is offering free trainings for individuals interested in becoming a hospice volunteer. For as little as 2-4 hours per week, volunteers can offer companionship, respite care and their own unique skills to patients and family members in their community. The first Hospice Volunteer Training is a comprehensive 8-week program, which will take place at the Portsmouth Public Library on 175 Parrott Avenue in Portsmouth, NH. The training will begin on Tuesday, September 12th from 1:30 pm – 3:30 pm and will continue every Tuesday until October 31st. Join us for this program that focuses on how to provide comfort, support and a reassuring presence to Hospice patients and their families. No medical or volunteer experience is necessary to make a positive difference.

In addition to hospice volunteering, Cornerstone VNA is also pleased to offer a unique volunteer opportunity that involves caring for the pets of hospice patients. As a new partner with the national Pet Peace of Mind Program, Cornerstone VNA is recruiting volunteers with an interest in offering pet care, such as dog walking, clean-up and other helpful tasks. With support from Pet Peace of Mind volunteers, patients are able to complete their end of life journey without worrying about their pet’s current or future needs. To become a Pet Peace of Mind Volunteer, Cornerstone VNA is offering 3-week training programs in two different locations. There will be a training at the Newmarket Public Library on 1 Elm Street in Newmarket, NH, which will begin on Monday, September 11th from 2:30pm – 5:30pm and will continue every Monday until September 25th. Another training will be held at the South Berwick Library on 27 Young Street in South Berwick, ME, which will begin on Thursday, September 14th from 1:00pm – 4:00pm and will continue every Thursday until September 28th.

Cornerstone VNA Looking for Volunteer Coaches for Balance Program

Cornerstone VNA is looking for volunteer coaches to assist in a new program called A Matter of Balance. This program is designed to help people manage concerns about falls and increase physical activity. Thanks to generous grant from the McIninch Foundation, Katie Michaud, MS, OTR/L of Cornerstone VNA, completed a two-day training session to become a Master Trainer to train coaches in A Matter of Balance. Once Katie has trained volunteer coaches, Cornerstone VNA will start offering 8-week classes to community members. Volunteer coaches will help participants become more confident about managing falls, help to identify ways to reduce falls, and lead exercises to help increase strength and balance.

No previous experience is needed to become A Matter of Balance Volunteer Coach and training is minimal. The training involves a two-day, 4-hour training session, with coach mentor, Katie Michaud. Once trained, volunteers will be assigned in pairs to facilitate community Matter of Balance programs that will take place over an 8-week period for 2 hours each week. Volunteers are asked to facilitate at least two Matter of Balance programs per year.

Cornerstone VNA Rehab Manager, Karen Krause, PT remarks, “Falls are on the rise as our aging population grows and it is something that goes undetected until it is too late. I am very excited about Katie becoming certified as a train the trainer for A Matter of Balance.” Krause adds, “Katie and I have started a community falls initiative and this will be a great adjunct to our program. A Matter of Balance is used widely in many other states and areas of NH and we are excited to be able to bring more classes to the communities we serve. With a drive to reduce falls in the community, we are excited to have a part in making this happen.”

Palliative Care Director Receives Vocational Award

Congratulations to Palliative Care Director, Kelly Doherty, RN-C, MSN, ARNP-BC, who received the 2017 Vocational Award from the Rotary Club of Portsmouth.  Four individuals were honored at the club’s luncheon on June 15th and Kelly was honored in the Health Care Category.

According to Katherine Berger, Transition Care Manager/Business Development Coordinator at Cornerstone VNA, “I was proud to nominate Kelly for this award and recognize her for her commitment to providing high quality palliative care as well as for her dedication to developing a successful program at Cornerstone VNA.  Since joining Cornerstone VNA in 2014, she has worked diligently to collaborate with local area hospitals, assisted living facilities and skilled nursing facilities in order to educate staff on the benefits of Palliative Care and how it can meet the complex needs of their patients.”

According to Julie Reynolds, RN, MS, CEO at Cornerstone VNA, “Kelly is truly passionate about her work and has certainly found her calling.  She has a special way of having honest conversations with patients and their families and providing exceptional guidance to them when having difficult conversations related to end of life decisions.  She often says that this is her dream job and has done a tremendous job growing her team and providing exceptional care to our patients and educating the community about the benefits of palliative care.”

Since the development of our Palliative Care program at Cornerstone VNA, many lives have been impacted in a positive way.  Kelly and her team work to ease concerns and guides patients and their families to identifying programs that are best suited for them, such as Hospice Care, Home Care or remaining on Palliative Care.

Cornerstone VNA Offers Personal Care Service Provider Training and Certification

Cornerstone VNA is offering a two-day training program for certification as a Personal Care Service Provider (PCSP). A PCSP is a person who performs and assists with the routine tasks of daily living for people with disabilities and special health needs. In general, a PCSP is someone who helps people live as safely and independently as possible.

The training will be on July 11 and 12 from 3:00 pm to 8:30 pm. Class instructors include Sandy Powers, RN, Donne Marchetto, Physical Therapist and Janice Howard, Director of Life Care-Private Duty. The cost is $40 and will take place at Cornerstone VNA, 178 Farmington Road, Rochester, NH. Participants will receive a certificate upon successful completion of this training program and are welcome to apply to be considered for a position at Cornerstone VNA as a PCSP.
