Cornerstone VNA Celebrates 105 Years

Cornerstone VNA is pleased to celebrate its 105th year and is proud to look back on more than a century of providing care at home.  In 1913, the Rochester Women’s Club, lead by president and philanthropist, Norma Snow, and the Ladies Aide of the First Church Congregational founded the VNA to provide homecare services.  Susan Gaudiello, Cornerstone VNA Board President states, “We are so proud of all the staff who have sustained the mission of the organization and cared for so many, all while keeping up with the changing health care needs of our community.”

According to Erika Lee, Director of Advancement, “I had the privilege of connecting with Norma Snow’s family in 2016 when we launched a new Legacy Society in honor of founder.”  Lee adds, “It was really wonderful to hear stories about Norma from her grandchildren and to learn more about this remarkable woman who continues to inspire us today.”

For more than a century, Cornerstone VNA has been committed to bringing services to people of all ages, so that families can stay together at home, even when facing the challenges of aging, surgical recovery, chronic or life-threatening illnesses or end of life care.  As a full service home care organization, Cornerstone VNA provides skilled nursing, rehabilitative therapies, social work as well as volunteer and support services through five distinct programs: Home Care, Hospice Care, Palliative Care, Life Care-Private Duty and Community Care.

Julie Reynolds, RN, MS, Chief Executive Office states, “Over the last century, times have changed from where and how care is provided and will continue to evolve.”  Reynolds adds, “I am certain that our founder, Norma Snow, would be quite amazed to see how far we’ve come and how advanced technologies and clinical specialties play an important role in the high quality care we provide to our patients each and every day.”

Cornerstone VNA cares for over 3,000 community members annually and provides support to their family members and caregivers.  In collaboration with local healthcare partners, Cornerstone VNA continues to work to ensure that quality care is at the forefront as they provide services to improve the health of the community and the patient experience.

Cornerstone VNA offers Palliative Care

Cornerstone VNA is committed to providing patients and their families with trusted, compassionate and expert care and has recently added a new Palliative Care Program to improve the care for patients living with a chronic or life limiting illness.  In addition to a Hospice program, Cornerstone VNA now provides palliative care consultations and pain management services to help patients manage the symptoms of their disease.  Palliative care is specialized medical care for people with a serious illness.  Its focus is on providing patients relief from the pain and anxiety of a complex illness, regardless of the diagnosis.  It is appropriate at any age and at any stage of a disease.  Palliative care is different from Hospice care in that it may start at diagnosis, and patients receiving palliative care may also receive curative treatment for their disease. The primary goals of the Palliative Care Program are to improve the quality of life for both the patient and their family and decrease the need for emergency room visits or re-hospitalizations.  Click here to learn more about Palliative Care.

Cornerstone VNA Offers Home Care

Home care consists of Skilled Nursing & Licensed Nursing Assistants

A licensed nurse visits a client in home to care for and monitor the medical condition and assist with special health care needs.  A Licensed Nursing Assistant assists clients with personal care, home exercises and activities of daily living. Cornerstone VNA Offers Hospice Care

Hospice is a holistic approach to caring for people who have terminal illness. It involves a team of trained specialists available 24 hours a day, who provide medical attention, pain and symptom management, emotional and spiritual support tailored to an individual’s needs and wishes. Hospice is not a place – it’s a concept of care that can be provided anywhere that a patient calls home such as in a private residence, nursing home, assisted living or hospital. Cornerstone VNA Offers Community Care

Cornerstone VNA provides health screening clinics monthly at a variety of locations and times noted below.

An RN will be available for blood sample screenings for diabetes, cholesterol, blood pressure monitoring, medication and diet counseling. The services provided are free, however, there is a small fee for a cholesterol screening and if noted. Cornerstone VNA Offers Life Care

What is Life Caretm?   Given a choice, home is where we want to be…for people of all ages.  But if we are experiencing health issues or growing older, being home alone can be more difficult, even challenging at times.  At Cornerstone VNA, our Life Caretm Program can provide a full array of services for you, from private duty skilled nursing care and rehabilitative therapies to assistance with activities of daily living, homemaking or simply the comforting presence of companionship.  Generally these services are private pay and are provided in addition to services patients receive under Medicare or private insurance.  If you have Long Term Care Insurance, coverage may be applied to the cost of most services.
