Kate Gagne, RN, CHPN, Achieves Hospice Nurse Certification

The Certified Hospice and Palliative Nurse (CHPN®) examination was designed for experienced registered nurses. Applicants must hold a current, unrestricted registered nurse license in the United States or Canada, and must have 500 hours of hospice and palliative nursing practice in the most recent 12 months or 1,000 hours in the most recent 24 months.

 

Hospice and palliative nursing practice is the provision of nursing care for the patient and family, with emphasis on their physical, psychological, emotional and spiritual needs when experiencing a life-limiting illness and through bereavement. This is accomplished in collaboration with an interdisciplinary team in a setting that provides pain and symptom management, family support and 24-hour nursing availability.  Cornerstone VNA cares for patients in private residences, assisted living facilities, nursing homes and hospitals, with a wide range of diagnoses such as cancer, dementia, stroke, COPD and heart failure.

Cornerstone VNA offers Personal Care Service Provider Training & Certification

Cornerstone VNA is offering a two-day training program for certification as a Personal Care Service Provider (PCSP).   A PCSP is a person who performs and assists with the routine tasks of daily living for people with disabilities and special health needs. In general, a PCSP is someone who helps people live as safely and independently as possible.

The training will be on May 24 and 25 from 3:00 pm to 7:30 pm. Class instructors include Sandy Powers, RN, Donne Marchetto, Physical Therapist and Janice Howard, Director of Life Care-Private Duty. The cost is $40 and will take place at Cornerstone VNA, 178 Farmington Road, Rochester, NH.   Participants will receive a certificate upon successful completion of this training program and are welcome to apply to be considered for a position at Cornerstone VNA as a PCSP. Cornerstone VNA Offers Successful Caregivers Connect Event

Cornerstone VNA proudly launched a new educational series last week titled Caregivers Connect, underwritten by Federal Savings Bank. Over 65 people attended the first session in the series, which took place on May 5th at the Governor’s Inn in Rochester. The afternoon included presentations from noted Alzheimer’s author A. Michael Bloom as well as Gerontologist, Dr. John Hopkins, owner and operator of Bellamy Fields and Watson Fields in Dover.

The session concluded with a panel discussion facilitated by Julie Reynolds, RN, MS, Chief Executive Officer of Cornerstone VNA, which included noted experts in the field including Dr. Hopkins, Michael Bloom, Sandy Powers RN, Wellness Nurse and Holistic Health Coach and Janice Howard, Life Care Director at Cornerstone VNA. All caregivers in attendance received a free copy of Michael Bloom’s book The Accidental Caregiver’s Survival Guide. Trained staff and volunteers from Cornerstone VNA provided on-site respite care to loved ones in a safe and welcoming environment, for caregivers who attended the event.

60% Decrease in CHF readmissions: Cornerstone VNA uses Health Recovery Solutions Tablets to Reduce Hospital Readmissions

Cornerstone VNA, a 4-star rated agency for quality patient care and patient satisfaction, has teamed up with the fastest growing telehealth company in home care to provide an array of unique monitoring features to complement its highly rated home care services.

Cornerstone VNA has partnered with Health Recovery Solutions (HRS), a software company started at Johns Hopkins University, to transform their telehealth remote monitoring program. HRS uses a 4G-enabled Samsung tablet with software to support education, self-care activities, and real-time videoconferencing and high-risk alerts. The tablet’s software teaches patients to take and record vital signs with the support of wireless Bluetooth devices. Patients answer disease specific daily symptom questions through the tablet. The state average for home care hospital readmissions for congestive heart failure (CHF) in New Hampshire is 16.7% and 16% nationally. Thanks to the new telehealth units, the 30-day readmission rates at Cornerstone VNA have decreased from 12.1% to 4.8%. That is a 60% reduction in CHF readmissions for patients that are on the telehealth program. Along with the 4.8% readmission rate, patients gave high ratings to the telehealth platform. On a scale of 0-5, the average score patients gave whether they would recommend the use of the tablet to a family member or friend was 4.08 (4-5 score means they strongly agree).
